Candidate Handbook
- Provides detailed information on requirements for licensure as a real estate, cemetery or membership
camping broker or salesperson. Forms, possible waivers and testing information included.

Applicant Clearance Card Team (ACCT) Supply Order Form
- Fingerprint Clearance Card Application kits order form.

- Candidates are issued the License Application by the testing company upon passing the state license
examination.

License Renewal Package
- Contains forms for renewal of salesperson and broker licenses (including employing [entity/sole
proprietorship], designated and associate brokers). LI-243 and LI-244, required; LI-400
and LI-555, required as applicable.

New Entity Checklist
- A list of appropriate forms and documents required for licensing a new entity.

New Entity Licensure Package
- Contains forms to create an entity and to appoint a designated broker. Please contact the
Department via e-mail at
Broker.appointments@azre.gov to schedule an appointment with a Broker Specialist. Forms
included are LI-212, LI-213, LI-214, LI-215 and LI-201.

ADRE User Agreement and AAR TM File Access Package
- Contains user agreement documents granting ADRE electronic access to User’s TM Program to enable
streamlined exchanges of information when ADRE conducts records inspections as authorized by
law. Documents included are ADRE User Agreement and ADRE AAR TM File Access Agreement.

Original Branch Office Package
- Contains forms to establish a new Branch Office. Forms included are LI-202 and LI-219.

- Use when applying for a license to operate a branch office.

Application for License Reinstatement (Form LI-207)
- To request reinstatement of a canceled or suspended license. Include Questionnaire (Form LI-244),
and Unlawful License Activity Statement (Form LI-555).

Application for LLC, Corporation or Partnership Entity Broker's License (Form LI-212)
- To apply for an employing broker's license for a Limited Liability Company (LLC), Corporation or
Partnership. Need to also include Aware Statement Form (Form LI-215).
- NOT used to become licensed through a Professional Corporation or Professional Limited Liability
Company.

Application for a Professional Corporation or a Professional Limited Liability Company (Form LI-231)
- Use this form for a Professional Corporation, (PC), or a Professional Limited Liability Company,
(PLLC) to;
- Create a new PC/PLLC
- Change a PC/PLLC by changing or a member or members
- Amending the name of a PC/PLLC
- To submit Termination or Articles of Dissolution
- Deleting a PC/PLLC

Application for a Temporary Broker's License (Form LI-240)
- The Commissioner may issue a temporary license as a broker to a licensed or unlicensed person for the
purpose of winding up the existing or pending business of a licensed broker in the following cases:
- To the surviving spouse, next of kin, the administrator or personal representative, or the employee of the administrator or
personal representative of a deceased licensed broker.
- To the spouse, next of kin, employee, legal guardian or conservator of a licensed broker disabled by sickness, injury or
insanity.

Application for Temporary Cemetery Salesperson License or Membership Camping Certificate of Convenience
(Form LI-227)
- Use to apply for a temporary cemetery salesperson's license or membership camping certificate of
convenience.

Aware Statement Form (Form LI-215)
- To be included with Application for LLC, Corporation or Partnership Entity Broker's License (Form
LI-212) and completed by the designated broker.

Broker Candidate Experience Verification (Form LI-226)
- To document licensed experience when seeking licensure as broker license. Information is
included in the Candidate Handbook.

Document Checklist (Form LI-400)
- Documents and information required for original license applicants, renewal and reinstatement
applicants and licensees making a disclosure required under A.A.C. R4-28-301 (A).

Employing Broker Change Form (Form LI-201)
- To make changes to the employing broker's license, including:
- For Designated Broker's (self-employed or for an entity) change of license status,
- An entity's change of designated brokers, or
- An employing broker's change of legal or DBA name or business address.

Final Checklist for New Entity (Form LI-216)
- Provides a submittal checklist for a New Entity. The checklist ensures that you have every
form and supporting documentation that you will need to have completed and in hand before you can
submit your request to the Department.

Licensee's Personal Information (Form LI-235)
- To provide confidential personal information, including social security number, date of birth, and
residential address, and to change residential address or residential mailing address, or legal
name. NOTE: Residential address changes can be filed online without charge. If requesting
the Department to change your residential address, a change fee applies.

License Renewal Questionnaire (Form LI-244)
- Use to provide information to determine your qualifications and suitability for renewal of an
Arizona license. Form LI-243 or LI-207, as applicable, MUST be included.

Original License Questionnaire (Form LI-214)
- For ALL original license applicants to provide information to determine applicant's
qualifications and suitability to hold an ADRE-issued license.

Renewal Application (Form LI-243) - part of the License Renewal Package
- Contains forms LI-243 and LI-244, required; LI-400 and LI-555, required as applicable.

Request for Administrative Severance (Form LI-224)
- For use by salesperson or associate broker to sever employment from employing broker when the
designated broker is not available to sign a sever form or refuses to sign it.

Request for Certification of Licensure (Form LI-217)
- To request a certification attesting to a person's licensure by the ADRE.
- Employment history is NOT included in certifications but is available from the Public Database on the
ADRE website.

Request for Legal and/or DBA Name Approval (Form LI-213)
- To request approval of and 30-day courtesy hold on a legal or DBA name when approved.

Request for Prelicensure Education Recertification (Form LI-245)
- Use this form to request approval for recertification of prelicensure education required under
A.R.S. § 32-2124-B. In order to qualify, your prelicensure education must be less than ten years
old and taken at a real estate school accredited by the Arizona Department of Real Estate.

Salesperson / Associate Broker Change Form (Form LI-202)
- To make changes to a salesperson's or associate broker's license status, including:
- Notice of employment by employing broker ("hire"),
- To sever employment from employing broker ("sever"),
- To change office location for same employing broker ("transfer"),
- To be licensed through a professional corporation (PC) or professional limited liability company
(PLC), or to change from being licensed through a PC or PLC,
- To change status from associate broker to designated broker (self-employed or for entity),
- To be appointed or removed as branch manager for branch office.
- NOT used to change personal information such as name or residence address.

Unlawful License Activity Statement (Form LI-555)
- Use if you are submitting a late renewal application or if you have conducted activities requiring a
current and active license while not properly licensed pursuant to A.A.C. R4-28-303.
