- Provides detailed information on requirements for licensure as a real estate, cemetery or membership camping broker or salesperson.  Forms, possible waivers and testing information included.

- Candidates are issued the License Application by the testing company upon passing the state license examination.

- A list of frequently asked questions and handy checklist to ensure that you have completed all the requirements for licensing or license renewal.

License Renewal Package - rev. 07/08
- Contains forms for renewal of salesperson and broker licenses (including employing [entity/sole proprietorship], designated and associate brokers).  LI-243 and LI-244, required; LI-400 and LI-555, required as applicable.

New Entity Checklist - rev. 03/08
- A list of appropriate forms and documents required for licensing a new entity.

New Entity Licensure Package - rev. 07/08

Form LI-226 - Broker Candidate Experience Verification - rev. 04/08
- To document licensed experience when seeking licensure as broker license.  Information is included in the Candidate Information Bulletin.

Form LI-227 - Application for Temporary Cemetery Salesperson License or Membership Camping Certificate of Convenience - rev. 03/04
- Use to apply for a temporary cemetery salesperson's license or membership camping certificate of convenience.

Form LI-231 - Application for Real Estate Salesperson or Broker Professional Corporation (PC) or Professional Limited Liability Company (PLLC) - rev. 03/08
- For licensure as a designated or associate broker or salesperson through a professional corporation (PC) or professional limited liability company (PLC) and to report changes regarding the standing, organization or members a PC or PLC pursuant to A.R.S. § 32-2125(B) and A.A.C. R4-28-303.

Form LI-235 - Licensee's Personal Information - rev. 03/08
- To provide confidential personal information, including social security number, date of birth, and residential address, and to change residential address or residential mailing address, or legal name.  NOTE: Residential address changes can be filed online without charge.  If requesting the Department to change your residential address, a $10 change fee applies.

Form LI-240 - Application for a Temporary Broker's License - rev. 08/00
- The Commissioner may issue a temporary license as a broker to a licensed or unlicensed person for the purpose of winding up the existing or pending business of a licensed broker in the following cases:

  1. To the surviving spouse, next of kin, the administrator or personal representative, or the employee of the administrator or personal representative of a deceased licensed broker.
  2. To the spouse, next of kin, employee, legal guardian or conservator of a licensed broker disabled by sickness, injury or insanity.

Form LI-243 - Renewal Application - part of the License Renewal Package - rev. 07/08
- Contains forms LI-243 and LI-244, required; LI-400 and LI-555, required as applicable.

Form LI-244 - License Renewal Questionnaire - rev. 05/08
- Use to provide information to determine your qualifications and suitability for renewal of an Arizona license.  Form LI-243 or LI-207, as applicable, MUST be included.

Form LI-400 - Document Checklist - rev. 11/07
- Documents and information required for original license applicants, renewal and reinstatement applicants and licensees making a disclosure required under A.A.C. R4-28-301 (A).

Form LI-555 - Unlawful License Activity Statement - rev. 05/08
- Use if you are submitting a late renewal application or if you have conducted activities requiring a current and active license while not properly licensed pursuant to A.A.C. R4-28-303.
